We are your patient-centered medical home.

The PCMH model adjusts how we deliver care to better serve patients and our community. It holds a promise of a new way to improve healthcare while fostering strong relationships with our patients.

Routine wellness and physical exams
Medication management
Preventative services including
Treatment of acute illnesses and injuries

Chronic Disease Management

Team-based care appointments with your primary care provider and pharmacist
Care planning and management of chronic health conditions
Comprehensive patient education and medication management

Lab Services

Certified Phlebotomy Technicians
Routine labs and blood work
Testing for certain acute conditions
HIV and Hepatitis C testing

Mental Health Counseling

Collaborative treatment plans and goals created by clients and their counselor
Variety of evidence-based therapeutic modalities offered
Eye Movement Desensitization and Reprocessing (EMDR) Therapy

Medication Dispensary

Affordable prescription copays
Patient Assistance Programs
Medication education counseling by a licensed pharmacist

Health Care for the Homeless is a Federally Qualified Health Center. We provide primary medical care and mental health services to individuals and families in Natrona County who are either homeless or at-risk of homelessness.

The mission of Health Care for the Homeless is to provide quality healthcare to individuals experiencing homelessness in Natrona County.
